While Large Language Models (LLMs) are making headlines, I believe Small to Medium-sized Language Models (SMLs) deserve more attention—especially for businesses operating in niche domains like healthcare. Why do I say this? 1️⃣ Tailored Solutions: In healthcare, where precision and specificity are critical, SMLs (40–50B parameters) are more adaptable to domain-specific needs. 2️⃣ Cost-Effectiveness: For startups, running and fine-tuning a SML is more economical than relying on large, resource-intensive LLMs. 3️⃣ Ownership & Flexibility: Open-source SMLs allow businesses to build and deploy models on their terms, giving greater control over data and performance. Therapist on Reddit: "I think that the best way to choose [an AI therapy note platform] is to open a few in separate tabs and start recording in all of them." I came across this comment when doing some market research for Quill Therapy Notes. This, uh, isn't what everyone's doing, right? My goodness, just think of how many third-party websites now have recordings of these (previously private) conversations! And does the client even know?* This is what keeps me up at night -- inevitably at least one of these platforms will have a data breach. * And even if the client "gave permission" and signed a consent form, did they truly understand the reason and risk factors? Did they truly have a choice, when being asked to sign something by their trusted therapist? (Again, this is why I am so opposed to recording sessions, and why Quill does not support that. The benefits do not outweigh the risks.)

What do you think about AI in senior care? Personally, I think we have to be careful with it. When AI presents itself as human, it can be confusing for seniors who may not have strong digital literacy skills. In fact, if they think they're talking to a person and not an AI chatbot, it can be dangerous if they're having a medical emergency or issue. However, there are some helpful use cases. I think Kristen Fischer's exploration of AI here is really interesting. "Artificial intelligence may be a useful communication tool to help older adults with cancer communicate with their doctors and have more of a say in their treatment," she says. AI can provide more context or explanation about certain treatment plans and translate the patient’s needs to clinicians in situations where the patient doesn't feel understood or heard. AI will never replace professionals, but it could help patients and clinicians better understand each other. Is an individual Enterprise LLM License a justifiable expense for your team members? I've seen this logic circulating: "If someone earning ≥$62,400/yr can save just one hour in a month using a #GenerativeAI tool, it justifies the cost of a $30/month #EnterpriseLLM." This assumes time directly equals productivity, and that a user doesn't already have an hour+ of free time in a month. We know this isn't always the case. Two quick factors to look at first: 1. The litmus test: Will time saved be reinvested into productivity? Is that productivity worth more or less than an hour of time on paper? 2. This simple formula: S>=max(C/H,F) In words: The number of hours saved (S) must be greater than or equal to either C / H (cost divided by hourly rate) or F (existing free time), whichever is larger. For the record, if we're using these tools effectively, an hour saved each month for is on the extreme low end of what I would expect for most business roles...

A few days late, but November was packed with big milestones for Stylo! We have a saying at Koa Health that Everything Product is Clinical and Everything Clinical is Product. While I can understand the sentiments here, and agree wholeheartedly that working in healthtech is different, I don’t feel posts like the one below are helpful. ✅ You of course should ALWAYS build an MVP - but minimally viable in healthcare requires ethics, safety, and compliance. ✅ Execute fast, Discover meaningfully. - things will and should be broken in our system, but the breaking requires close guidance and thoughtful approaches WITH clinicians. ‼️PLG is definitely more tricky in Healthtech but I don’t understand why this is even called out here. It’s irrelevant how growth should or should not happen as long as it’s ethical and in the best interest of health outcomes. 💡I can get on board with investing in the support of your users more than traditional tech - but again… I don’t see why this is a general statement about healthtech products. It’s based on the needs of your users. If tech can facilitate it effectively and safely, go for it. ✅ stages aren’t bad. Over promising and making unproven claims on stages is bad. Let’s focus on the root problem, not the forum.
